Immunization Resources & Information

For scholars in needed of updated vaccination. Document includes requirement and location of brooklyn based vaccination center

25-26 Health Examination Form / Physical

An updated physical is MANDATORY for all New York City School aged scholars, once a year. Must be returned to school.

25-26 Allergy & Anaphylaxis Admin Form

Needed for scholars with allergies (ie. Food, Allergens, Seasonal or need for Epi-Pen) (optional/ only if necessary)

25-26 Asthma Administration Form

For scholars that require Asthma medication to be administered by school nurse (optional/ only if necessary)

25-26 Diabetes Medication Administration Form

For scholars that require Diabetic medication to be administered by school nurse (optional/ only if necessary)

25-26 General Medication Administration Form

For other parent provided & doctor instructed medications that need to be administered by school nurse (optional/ only if necessary)
